How do I find the Surgical Artistry page on FaceBook?

This is confusing because somehow FaceBook has a page created for Surgical Artistry – but regardless of how I try to “edit” that page – it doesn’t take my “suggestion” for redirecting to a page we created – which we consider the “official” Surgical Artistry FaceBook Page.

However after a while, some of my patients thought I (Calvin Lee, MD) should have a page of my own relating to Botox, Juvederm, and other Dermal Fillers.  So we created:  www.FaceBook.com/BotoxModesto.  On this page we talk about things like cannulas for cosmetic injections, dermal fillers for lip augmentation, and perhaps even cosmetic acupuncture.  this page is more related to the stuff I do, but with a slant on cosmetic injections and dermal fillers.

Modesto Breast Augmentation Informational Session with Dr. Tammy Wu

Cosmetic Breast Surgery Open House at Surgical Artistry

Saturday, June 1, 2013. (this event has passed, but if you are interested in more events like this with us in the future, feel free to let us know).  Of course you can still have a private breast enhancement surgery consultation with Dr. Tammy Wu.

Small group discussions relating to Breast Enhancement with Dr. Tammy Wu – which includes Breast Augmentation and Breast Lift. Get some expert information and meet Dr. Tammy Wu in person. This is an introduction to a formal one-on-one consultation with Dr. Tammy Wu which may be scheduled for a future date.

Come anytime between 9am-12pm on Saturday, June 1, 2013.  We’ll have free food and prizes. Free informal small group meetings with Dr. Wu at 9am, 10am and 11am (pick the time that works for you). Existing patients – feel free… to come and bring friends and of course enter the free drawing for 8 winners:The 8 winners will get to choose: a box of 3ml Latisse (normally $89 at Surgical Artistry – retails for $120) or 20U of Botox (normally worth $234 at Surgical Artistry). Entry rules: You just have to stop by long enough to fill out a drawing form. Dr. Tammy Wu is a board certified Plastic Surgeon who graduated top of her Ivy League medical school – Brown University. She was given the award for the #1 graduating female medical student. She has been in practice in Modesto, CA for 10 years. Her husband is Dr. Calvin Lee, a board certified General Surgeon who specializes in Botox, Fillers (Juvederm), Veins, and Acupuncture and he will also be at the informal informational session to provide any information regarding his areas of experience.

  • About 40-50 people came.
  • The event was staffed and sponsored by Mentor, makers of breast implants.

Lidocaine Info Important For My Surgical Practice – Maximums and Toxicity

Lidocaine

  • Also known as Xylocaine and sometimes Lignocaine
  • Local Anesthetic
  • Antiarrhythmic drug – for ventricular arrhythmias (but Amiodarone is mostly used for this)
  • The first Amino-Amide local anesthetic
  • First marketed in 1949
  • Alters signal conduction in neurons by blocking the fast voltage gated sodium channels in the neuronal cell membrane.  Thus prevents depolarization of the postsynaptic neuron.

Esters and Amide local anesthetics – way to remember which

  • Esters have only one “i” in the name: Procaine, Cocaine
  • Amides have over two “i”s in the name:  Lidocaine, Bupivicaine, Prilocaine.

Contraindications to using Lidocaine

  • Porphyria – rare inherited or aquired disorders of enzymes which produce porphyrins and heme resulting in neurological complications (abdominal pain, vomiting, neuropathy)  or skin problems.
  • Heme is one the best known porphyrin the pigment in red blood cells and cofactor of the protein hemoglobin.
  • Porphyrin means purple

Lidocaine overdose

  • can occur with topical creams
  • Treatment with IV lipid emulsions can reverse the effects of lidocaine toxicity.

Adverse Effects

  • Rare
  • Allergic reactions rare.
  • CNS (central nervous system) and cardiovascular effects
  • CNS at lower doses, cardiac at higher doses
  • CNS: nervousness, tingling around mouth, tinnitus, tremor, dizziness, blurred vision, seizures, depression, loss of consciousness
  • Cardiovascular effects: hypotension, bradycardia, arrhythmias

Insensitivity to Lidocaine

  • Genetic
  • Exists in some patients with ADHD and Ehlers-Danlos syndrome

Lidocaine Safety and Toxic doses

  • Particular care in calculating maximum safe dose for young children
  • Peak blood levels of lidocaine usually occur 10-25 minutes after injection – the point at which toxic effects are likely to be seen.
  • Maximum safe dose of lidocaine without vasoconstrictor (ie. epinephrine) is 3-4.5 mg/kg (duration of action 1 hour).  No more than 300 mg (30 cc’s of 1% Lidocaine) at once.
  • Maximum safe dose of lidocaine with vasoconstrictor is 5-7 mg/kg (duration of action 2-6 hours)

How to calculate Maximum safe doseage of Lidocaine

  • You need to know that 1% Lidocaine solution is 10 mg/mL
  • Example Calculation for a 70 Kg man:  Lido 1% with Epi:  keep under 7 mg/Kg.  7×70 = 490 mg for 70 kg man.  Divide 490 mg by 10 mg/ML = 49 ml.

How is this important to our Plastic Surgical and Acupuncture practice?

  • We use lidocaine in plastic surgical procedures – topical and subcutaneous – for removal of moles, injection of fillers such as Juvederm, and sometimes for Botox.
  • I also use injected  lidocaine as part of some trigger point deactivation practices in acupuncture.

Cosmetic Procedures at Surgical Artistry which may use Lidocaine

  • Liposuction – tumescent lidocaine
  • Mole removal – injected subcutaneous local lidocaine
  • Fillers – Juvederm – some formulations have lidocaine within the product
  • Fillers and Botox – sometimes we use topical lidocaine
  • Face Veins – sometimes we use topical lidocaine
  • Minor office surgeries – we use injected lidocaine in the subcutaneous level

I get this question a good deal.  I think it’s probably somewhat confusing as to what we do and we offer a lot of procedures and products.  Basically we are a two surgeon medical group offering services in Veins, Acupuncture, Plastic Surgery, and General Surgery.  That can be quite a lot.  But even with that said, it’s somewhat confusing as to what each of these items involves.  So I thought I’d make a list which doesn’t include everything, but covers most of what we do:

Senate Resolution Award for Surgical Artistry from Senator Tom Berryhill

Senator Tom Berryhill made a surprise visit in person to our office in April 2013 and we took this picture holding up the Senate Resolution signed by him, commending Surgical Artistry! Because we were totally surprised by the award, I was given this very nice explanation of the award: “This award was given because of what you and Dr. Wu have done for the people of Modesto! These awards are not given lightly. There is a process that must be gone through and certain criteria that must be met before an award of this type is even considered. You both earned this through all of the selfless, tireless hard work you do every day of the year. The Senator Tom Berryhill’s Sacramento staff did their research and felt you deserved this Senate Resolution.”

Bob Phelan gave me the award

while I was pigging out at the pasta carbo loading feast the night before the Surgical Artistry Modesto Marathon.  Bob is the District Director at the California State Senate and a great friend of ours.  We sat at the same table during the carbo loading dinner but he seemed a little too well dress and had a name tag on.  Well, I figured he was there to make a political appearance and I figured he had to work that night.  When he was called up to make a speech, I started chowing down at full speed.  I might not run fast, but I can eat fast. Then before you knew it he called my name to come up to receive an award.  Boy was I in for a surprise!   Thank you Bob for working that night.  What he didn’t know was that I was stifling some hiccups while up there on the stage while getting these picture taken:

Bob and his wife Alta also participated in the Surgical Artistry Modesto Marathon.  It was certainly a pleasant surprise.  Alta recounts the funny look on my face when I had to suddenly stop eating to go up on stage.  She thought it was worthwhile, but there was no picture of that – thank goodness.

Dr. Wu couldn’t be there because she had to rush off to a meeting in Sacramento, and then she was coming home later that night to get ready for the event the next day (The Surgical Artistry Modesto Marathon).  But while she was driving home from Sacramento, I updated her on the exciting news of the award, and she was very impressed with the wording on the framed Senate Resolution Award.

got a chance to meet and the above (on top of the blog) photo was taken.  And several of these other photos were taken.  It was very considerate and we were very honored to have Senator Tom Berryhill visit our office (yet another great surprise!) along with Bob Phelan too.

This is just the beginning

We are proud founders of the Gallo Center for the Arts, founding title sponsors for the Surgical Artistry Modesto Symphony Pops Series / Surgical Artistry Modesto Symphony Fat Cat Classics 2006-2009, and founding title sponsors for the Surgical Artistry Modesto Marathon.

The award hangs up proudly at our office near the entrance and it serves as a reminder to us that this is just the beginning of what we set out to do.  We want to do great things for our community here in Modesto, California.  Thank you for the recognition and we hope to continue with our efforts which include performing surgery at a very high level.  And of course that includes a continued effort on continuing our life long learning in medicine.
